INTRODUCTION Maternal mortality is the leading cause of death among women of reproductive age in developing countries (1). Although great gains have been made in the past two decades in reducing childhood morbidity and mortality and in improving reproductive health in general, significant reductions in maternal mortality have been elusive (2). Technical improvements are essential to improve birth outcomes in many settings; however, more distal influences, such as the larger sociocultural environment, have important impacts on the decision to seek professional care (3).
